Your body’s natural estrogen and progesterone levels decline during perimenopause and postmenopause. These changes to your hormonal environment are key drivers behind the spectrum of perimenopause and menopause symptoms like hot flashes, sleep disturbances, and mood changes. HRT replaces those declining hormones to help relieve menopause symptoms.

The type of HRT your receive depends on your medical history:

  • Choose estrogen-only therapy (ET) if you’ve had your uterus removed (hysterectomy), as this eliminates the risk of unopposed estrogen thickening the uterine lining.
  • Opt for combined estrogen-progesterone therapy (EPT) if you still have a uterus, as progesterone counteracts estrogen’s effect on the uterine lining and protects you from endometrial cancer risk.

Why do people need different doses of HRT?

Your healthcare provider will tailor your HRT dosage based on several factors, including:

  • Symptom severity: People experiencing stronger vasomotor symptoms (VMS) like hot flashes or night sweats often require higher starting or maintenance doses to find relief.

What triggers a dose change?

But even for product-specific dosing categories, doctors make dosage decisions at the level of the individual. There’s no single “right” dose of estrogen for menopause care. Here are the types of estrogen and their typical dosages.

Oral estrogen

When taking oral estrogen, you swallow a tablet or capsule. The drug absorbs through your digestive tract and travels to your liver before entering general circulation. Because your liver acts as your body’s primary filter, it begins breaking down the medication before it reaches your wider bloodstream. The liver also converts a large percentage of estrogen to a weaker form of estrogen called estrone. This process reduces how much of the drug reaches your bloodstream. That’s part of why oral estrogen often comes at higher milligram doses compared to forms that bypass the liver. 

In oral formulations, two types of estrogen are common: estradiol and conjugated equine estrogens (CEE):

  • Estradiol: 1 mg to 2 mg standard dose per pill; 0.5 mg is typically considered low-dose
  • CEE: 0.625 mg is standard; 0.3 mg is low-dose (ranges vary)

Transdermal estrogen

With transdermal estrogen, you apply the drug topically via patches, gels, or sprays. All forms of transdermal estrogen pass through the skin, bypassing the liver’s filter, before entering your bloodstream. Because more of the drug reaches circulation, transdermal estrogen doses are often lower than oral formulations.

Patches are the most common form, with a 0.05 mg typical standard dose. For low-dose therapy, 0.025 mg is common and roughly equal to 1 mg of oral estrogen. Dose ranges vary across the spectrum of transdermal estrogen types. 

Local vaginal estrogen

You apply local vaginal estrogen directly to vaginal tissue as a cream or tablet. Doctors typically prescribe this type of estrogen to treat genitourinary symptoms of menopause (GSM). These include experiences like vaginal dryness, recurrent UTIs, and painful sex. 

Local vaginal estrogen doses are typically lower than transdermal and oral alternatives. Dose grading systems don’t map well to these products because they’re usually low-dose by design. You can expect an estradiol vaginal tablet to sit somewhere in the vicinity of 10 mcg, but this can vary. 

Combination HRT doses and delivery routes

If you still have a uterus, your treatment generally can’t rely on estrogen alone. You may be able to use local vaginal estrogen alone, as very little enters your bloodstream. However, for oral and transdermal estrogen, you’ll need a progestogen to protect your uterine lining. Like estrogen, combination HRT comes in low-dose and standard-dose options.

Oral combination HRT

Just like oral estrogen alone, oral combination pills must pass through the liver first before entering circulation. The liver also breaks down a significant portion of the progestins, so these medications require higher doses. 

Oral pills typically use synthetic progestins like norethindrone acetate or levonorgestrel. Doctors prescribe them as a continuous-combined regimen, where you take a standard or low dose of both hormones every day to prevent monthly bleeding. A low-dose option might pair 0.5 mg of oral estradiol with 0.1 mg of norethindrone, while standard doses can scale up to 1 mg of estradiol and 0.5 mg of norethindrone.

Transdermal combination HRT

Transdermal combination therapy uses a skin patch that continuously delivers both estrogen and progestin directly through your skin and into your bloodstream. Like transdermal estrogen, this bypasses the liver’s initial filter, allowing for you to use a lower dose to achieve similar symptom relief. 

Patches typically use synthetic progestins. You change the patch once or twice a week, and it delivers 0.05 mg of estradiol and 0.14 or 0.25 mg of progestin.

Separate estrogen and micronized progesterone HRT

Rather than using a single pill or patch that combines both hormones, some doctors prefer to prescribe transdermal or oral estrogen alongside oral micronized progesterone. 

Oral micronized progesterone is bioidentical to the progesterone your body naturally produces, meaning it has the same molecular structure. Synthetic progestins are molecularly similar, but they’re not an exact match for your body’s progesterone. Because of this, synthetic progestins sometimes accidentally bind to similarly shaped testosterone or glucocorticoid receptors, causing side effects like acne, bloating, and mood swings. Bioidentical micronized progesterone doesn’t have that issue.

You can take separate estrogen and micronized progesterone daily, using one of the above doses of oral or transdermal estrogen combined with a 100 mg micronized progesterone pill. Or you can take this regimen cyclically, using estrogen daily but taking a higher dose of micronized progesterone (usually 200 mg) for only 12 to 14 days out of the month. This mimics a natural menstrual cycle, triggering a monthly period.

What’s the optimal HRT dose for menopause?

The FDA previously emphasized using the lowest effective HRT dose for the shortest amount of time. However, in late 2025, the FDA updated its longstanding stance. Now, there’s a greater emphasis on personalized care where doctors tailor treatment to the individual. Low-dose HRT is appropriate for some women, while higher standard doses may produce greater therapeutic outcomes for others.

A study published in Obstetrics & Gynecology measured four estradiol doses (0.25 mg, 0.5 mg, 1 mg, and 2 mg) against a placebo across 333 menopausal women. They compared these doses’ efficacy for against VMS like night sweats and hot flashes:

Increased dosage also influences HRT’s safety and risk profile. Your doctor will work with you to proactively adjust your dose as needed throughout your treatment.

Side effects

A Cochrane systematic review pooled data from three randomized controlled trials. Researchers assessed how likely participants were to report adverse effects like headache or breast tenderness across oral estradiol doses compared to placebo. They used odds ratios (ORs) to measure this, where anything above 1.0 indicates higher odds of side effects:

This data suggests that the higher the dose, the greater the odds of experiencing side effects compared to a placebo. However, these figures have limitations, including a small sample size.

Serious risks

Dose can also correspond with more serious risks. For example, in some cases, high-dose oral estrogen is tied to greater relative blood clot risk, but not a greater absolute risk. Greater relative risk tells you how much a higher dose multiplies the chance of an event compared to a control group. Absolute risk tells you the percentage jump in real numbers. In this case, the relative risk increases by two to three times, while absolute risk increases from one to two cases per 1,000 to two to four cases per 1,000.

For breast cancer, estrogen-only therapy has a minimal effect on absolute risk, even with extended use. Relative risk remains close to baseline, with absolute risk increasing by two to nine cases per 1,000. However, with combined HRT (estrogen plus progesterone), there’s a greater relative breast cancer risk. Long-term combined therapy can increase relative risk by 1.5–2 times, while absolute risk results in an additional six to 18 cases per 1,000 over a multi-year window.
