‍PRODUCT LINE DESCRIPTIONS

All available Product Lines are described below. Specific Product Lines actually being purchased by Customer are identified on the Order Form’s Fee Schedule.

PRODUCT LINE

Maternity

  • Up to 21-month end-to-end program supporting expecting parents during pregnancy and for 12 months after birth through the postpartum and return-to-work periods.
  • Program includes Pregnancy, Partner, and Loss and Pregnancy Options tracks, and a Participant may move from one track to another within the same Maternity Product Line enrollment up to 12 months postpartum.
  • Care advocacy and navigation support, including referrals to in-person OB-GYNs.
  • Virtual practitioner network across 30+ specialties, such as OB-GYNs, Doulas, Career Coaches, Lactation Consultants, Mental Health Specialists, and Sleep Coaches.
  • Content library of educational resources, wellness tips, and baby development tracking.
  • Community forums for interactions with peers and virtual practitioners.
  • Virtual group classes and videos, such as Breastfeeding 101, Infant CPR, and Newborn Care.
  • Available product add-on: Breastmilk shipping program, Maven Milk, to support breastfeeding employees as they return to work (see description below).

Family Building: Fertility, Adoption, Surrogacy, Fertility Preservation

  • 12 month renewable programs delivering end-to-end care, including education and 24/7 care advocacy and support.
  • Educational content and coaching from reproductive health and fertility experts across 30+ specialties to achieve health goals and explore paths to parenthood.
  • Community forums for interactions with peers and virtual practitioners.
  • Virtual group classes and videos.
  • Career coaching around family planning in the workforce.
  • Referrals to Maven’s clinic and pharmacy partners (as applicable).
  • Access to negotiated discounts with Maven's clinic and pharmacy partners.
  • Available Product Lines:
    • Fertility (12 months): Holistic approach fertility, promoting overall health literacy for women of reproductive age. End-to-end care from preconception through considering or undergoing fertility treatments, including education, 24/7 care advocacy and support, and support choosing a high-quality clinic.
    • Adoption (12 months): Personalized coaching, communities, and care supporting families throughout their adoption journey.
    • Surrogacy (12 months): Personalized coaching, communities, and care supporting families throughout their surrogacy journey.
    • Fertility Preservation (12 months): Support and care for members through the considerations and process of fertility preservation, either for medically necessary care or elective care, including egg or sperm freezing.

Maven Wallet

  • Maven Wallet is a reimbursement and 12-month renewable program that can be activated by a Participant enrolled in a corresponding eligible Maven Program Product Line. Enrollment fees are charged at the time a Participant enrolls and annually thereafter.
  • Wallet-enrolled Participants have access to Maven Wallet’s administrative reimbursement services through a platform that allows each Participant to a) aggregate the collection of required documentation for reimbursement of eligible expenses, or b) to pay for covered services in accordance with the Customer’s applicable employee benefit plan rules.
  • Each Participant’s access to administrative reimbursement services shall commence upon the Participant’s activation of Maven Wallet in the platform and shall end upon the earlier of: a) when Participant reaches the Individual Reimbursement Maximum; b) 90 days after termination of this Agreement; or c) 90 days after termination of the Participant’s employment as reflected by their removal from the then-current Eligibility File or otherwise communicated by Customer in writing.
  • The Maven Card is a debit-like card that provides an alternative payment method for Wallet-enrolled Participants in lieu of using a Participant's personal form of payment prior to submitting a request for reimbursement through the Maven Wallet platform. The Maven Card can be used by Wallet-enrolled Participants who are part of certain Family Building tracks as identified by Maven. Participants electing to use Maven Card must substantiate all eligible expenses that are paid using the Maven Card and are subject to all terms and conditions of the Maven Wallet program.
  • If Customer elects to offer the Maven Card, Wallet-enrolled Participants will be prompted to request a Maven Card through the Maven app and a card with activation instructions will be mailed to the Participant. Participants on a High Deductible Health Plan that contribute to a Health Savings Account will need to meet minimum statutory deductibles prior to being able to use the Maven Card for payment of eligible medical expenses. Use of Maven Card by Wallet-enrolled Participants is not mandatory; in lieu of using the Maven Card, Participants can seek reimbursement for eligible expenses by paying out of pocket and submitting a request for reimbursement through the Maven Wallet platform.

    Maven Milk

    • Breastmilk shipping and travel solution, providing nursing parents with an easy, reliable, and safe way to get their milk home to baby when traveling.
    • Each available Maven Milk product (as described below) has a separate Enrollment Fee. Customer shall be responsible for paying an Enrollment Fee for each product ordered by a Participant.
    • The Maven Milk Products are only offered to Customers who purchased Maven Maternity.
    • The Maven Milk Products offered to Participants, which may be updated from time to time, are described at https://www.mavenclinic.com/maven-milk and currently include the following (each a “Maven Milk Product”):
    1. “Pump & Post Kit” Ground shipping of cooler to Participant and expedited shipping of cooler from Participant to baby at address specified by Participant. (Available in the U.S. only)
    2. “Pump & Carry Pack” Ground shipping of reusable foam ice packs & tote to Participant.
    3. “Pump & Check Kit” International shipping of foam ice packs and carry on cooler.
    • Each of shipment of one Maven Milk Product is considered one enrollment in the Maven Milk Line (e.g., if a Customer Beneficiary is going on a multi-night business trip and orders three Pump & Post Kits, that would be considered three enrollments with three Enrollment Fees).
    • The Participant’s access to services in the Maven Milk Line of the Program shall commence upon enrollment with Maven in accordance with this Agreement and shall end the earlier of: 24 months postpartum, when this Agreement expires or terminates, or when the Participant ceases to be an Eligible Customer Beneficiary (as indicated via the Eligibility File or otherwise by Customer in writing).
    • Any incremental shipping costs, over what is covered in the Maven Milk as described above, will be billable to the Customer.

    Parenting & Pediatrics

    • 12-month renewable program offering integrated coaching for parents and specialized pediatric care.
    • 24/7 access to virtual pediatric care including developmental psychologists, occupational therapists, and special needs advocates.
    • Content and classes specific to each stage of childhood development to build stronger parent-child relationships and decrease behavioral issues.
    • Community forums to connect with other parents and share experiences and tips.
    • Career coaching and concierge service to recommend in-person providers and help navigate childcare options, improving employee retention.

    Menopause & Midlife Health

    • 12-month renewable program promoting overall health and wellbeing for anyone experiencing menopause or in midlife.
    • Care management to identify and support symptoms throughout a member’s midlife health journey, including menopause, hormonal changes, chronic conditions, and mental wellness.
    • Virtual specialists available 24/7—including OB-GYNs, men’s health nurse practitioners, nutritionists, career coaches, and more—to provide personalized treatment plans, prescription management, and holistic care for lasting health and career longevity.
    • Referrals and navigation to additional employer-sponsored resources and high-quality in-person providers.

    Maven Managed Benefit

    • Coverage for specified services is transitioned from current plan coverage (if applicable) and to Maven to administer.
    • Maven administers employer-sponsored benefit coverage based on Customer’s selected program design. Participants pursuing fertility-related treatment receive care through the Maven Performance Network of clinics. If applicable, and subject to the applicable health plan agreeing to accept the accumulation of this carved out spend, Customer will facilitate and Maven will set up data feeds with sponsored health plans to accumulate Participant’s fertility-related medical spend towards their health plan deductible/out-of-pocket maximum. Maven’s direct payment model allows Maven to facilitate payment of Participant cost share and Customer financial responsibility and pay Maven Performance Network clinics directly for covered fertility services rendered. Through our in-app Maven Wallet product, Participants are able to track their employer-sponsored coverage.
    • Access to Maven’s preferred network of third party Fertility Pharmacies. Participants can choose to use a different pharmacy and pay upfront for their prescription, and then seek reimbursement from Customer for the Customer cost share pursuant to the employer-sponsored benefit coverage.
