What’s Foundayo®?

Foundayo® is the brand-name medication for orforglipron, manufactured by Eli Lilly. Orforglipron belongs to a class of medications called glucagon-like peptide-1 receptor agonists (GLP-1 RAs). This class mimics a natural hormone called glucagon-like peptide-1 (GLP-1). 

Foundayo® influences the body in several key ways: 

  • Insulin secretion: When blood sugar levels rise after a meal, orforglipron tells your pancreas to release insulin. This helps move blood sugar out of the bloodstream and into cells, where the body uses it for energy. 
  • Glucagon suppression: Glucagon (a hormone) tells the liver to release stored blood sugar when your levels run low. Orforglipron suppresses glucagon release, helping prevent unnecessary increases in blood sugar. 
  • Gastric emptying: Orforglipron slows how quickly food leaves the stomach after eating, which prolongs feelings of fullness and suppresses post-meal blood sugar spikes. This is referred to as delayed gastric emptying. 
  • Satiety: Orforglipron acts on areas of the brain that regulate appetite, such as the hypothalamus. Activating these areas suppresses appetite, helping reduce the amount of food you consume.

Who can take Foundayo®?

Foundayo® is FDA-approved for chronic weight management, in combination with a reduced-calorie diet and increased physical activity. It’s for adults experiencing:

  • Obesity, defined as a body mass index (BMI) of 30 or higher
  • Overweight, who also have at least one weight-related condition
  • Weight-loss maintenance needs after stopping semaglutide or tirzepatide
  • Any menopause stage, in conjunction with weight qualifications

Foundayo® isn’t approved for adolescents. 

Foundayo®: Serious adverse effects (SAEs)

Even if you meet Foundayo®’s baseline eligibility threshold, that doesn’t mean it’s automatically right for you. Before prescribing, your doctor will screen for risk factors. Contraindications are a key part of that equation. These are the conditions or circumstances that could make a medication unsafe for you.

Foundayo® carries two contraindications:

  • Thyroid cancer: A personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2)
  • Allergic reaction: A past serious allergic reaction to orforglipron or any other of Foundayo®’s ingredients

Foundayo® carries additional warnings (not contraindications). It’s not recommended for patients with severe gastroparesis or who are breastfeeding, for example. Your doctor will discuss all potential risks with you.

The benefits: Clinical study on Foundayo® for weight loss 

A phase three trial titled ATTAIN-1, published in the New England Journal of Medicine, centered around one key question: Is Foundayo® effective for weight loss? For this 72-week trial, researchers randomly assigned 3,127 eligible participants to a placebo or one of three orforglipron doses (6 mg, 12 mg, or 36 mg*). People took medication once daily. Here’s how much weight they lost (of starting body weight), on average:

  • 6 mg: 7.5% (2.1% placebo)
  • 12 mg: 8.4% (2.1% placebo)
  • 36 mg: 11.2% (2.1% placebo) 

Notably, 18% of participants in the 36 mg group lost at least 20% of their starting body weight, compared to just 2.8% on placebo. Researchers also observed additional benefits beyond weight loss. These include reductions in systolic blood pressure, triglycerides, and non-HDL cholesterol—all positive cardiometabolic indicators. 

Several parts of this study influenced how Foundayo® is prescribed today. The doses from the trial are comparable to today’s three maintenance doses: 5.5 mg, 9 mg, and 17.2 mg. ATTAIN-1 used capsules, while Foundayo uses tablets. These forms vary in their bioavailability (how much of the medication reaches the bloodstream). A 36 mg capsule of orforglipron, for example, is closely comparable to a 17.2 mg tablet.

How do you take Foundayo®?

Foundayo® regimens involve taking one tablet, once daily, at any time. You can take it with or without food. As you start treatment, your doctor will guide you through titration—gradually increasing your dose over time, allowing your body to adjust to treatment. 

The standard Foundayo® titration process looks like this: 

  • Month one: 0.8 mg
  • Month two: 2.5 mg
  • Month three: Find your maintenance dose (5.5 mg, 9 mg, 14.5 mg, or 17.2 mg)

Reaching your maintenance dose means working through each step before it. So if you’re building toward 17.2 mg, you’ll move through each lower dose first, waiting at least 30 days between increases.

What are the side effects of Foundayo®?

In ATTAIN-1, up to 10.3% of participants discontinued treatment due to side effects (dose-dependent), compared to 2.7% on placebo. Gastrointestinal (GI) side effects were the most common, which include nausea, constipation, and diarrhea.
